Key takeaways

  • Verified means source, method, date, outcome, and proof—not a vague checkbox
  • Discrepancies should become owned work items
  • Re-verification dates keep the record alive
01

What PSV actually is

Primary source verification is the process of confirming a provider credential directly with the issuing source or its approved channel. It is not a scanned copy sitting in a folder, and it is not a verbal confirmation with no audit trail.

Teams get into trouble when “verified” means different things to credentialing, medical staff, and compliance. Align on a definition before you argue about volume.

02

What a useful verified status includes

A durable verified record should capture enough to stand up in review:

  • Issuing source or approved channel
  • Method used (portal, letter, API, board site)
  • Verification date and outcome
  • Supporting proof artifact
  • Next re-verification date
03

Treat discrepancies as work

When names, numbers, or dates do not match, the discrepancy is operational work—not a sticky note. Assign an owner, capture the blocker, and track resolution until the credential can honestly move forward.
